Assuming, you've done "Refresh" in the browser, please check whether "C:\Program Files\UltiDev\Cassini Web Server Explorer\images" folder contains
banner.gif (or if you are on the machine where Cassini Explorer is installed, please click the link). If image is not there, please try reinstalling UltiDev Cassini. You can also take a look at Windows Event Log (Control Panel -> Administrative Tools -> Event Viewer -> Application) and see whether there are any error entries there. If there, please post them in this thread and we'll take a look.
